Media Release Forms

Purpose

The WVU Extension Service Media Release Forms authorizes the use of an image of, sound recording of or words attributed to you, a minor, or an individual at any event whom you would like to interview or be interviewed.

Media is something that may be seen or heard by the public, including:

• Posters/displays/presentations
• Educational reports
• Television
• Newsletters/magazines
• Videos
• Internet
